Once the Fingerprint Request Form is completed, please either fax to 503-925-7159, e-mail to burkes [at] sherwoodoregon.gov (burkes[at]sherwoodoregon[dot]gov), drop by or mail to the City of Sherwood Police Department. You will be contacted and an appointment set up. Cost is $29.18 per card.
